Beginning

Those items should allow for enough hp for surviving and damage for last hitting. The 3 different sources of health regen are for possible heavy harass lanes. 1 point on Heartstopper Aura to hinder enemy regen. No mana regen needed because of Sadist, if you're getting last hits you have mana.

Make your choice on being aggressive or passive in lane, I would choose to be offensive/aggressive because if Death Pulse is timed correctly it gets last hits, which returns some mana, and harasses enemies.

All of the laning items can be bought from the sideshop with the exception of the Magic Wand.

Laning

One thing that you need to remember is that if you are going this style you NEED to get a LOT of last hits. This is not a build for unskilled last hitters, if you cannot last hit consistently then DO NOT DO THIS BUILD. Use Death Pulse often to harass once you have Sadist but only at times where you can both last hit with your "Q" skill AND harass. Trust me, a lot of times you will be playing aggressive so that you are able to farm and this will help your lane very much.

You can also choose to not get Heartstopper Aura at all and just go straight for Sadist & Death Pulse combo for lots of harass and easier mana regen at early levels. Although I do not do this personally, some choose to do so because they don't like the aura skill.

BE CAREFUL
I cannot stress this enough, if you do not have a great aggressive lane partner with a stun then you should definitely be defensive. Over extending with this low hp hero may cause you a few deaths and then you are no longer that strong, tanky carry that you should be.

Mid game

Keep farming and pushing lanes hard. Try to farm Bloodstone as fast as possible because if you are near the action getting kills and helping the team then you are getting stronger and stronger. Next item after this is entirely dependent on the enemy team.

Heavy magic damage Hood of Defiance - goes without saying, more magic resist helps survival
Heavy melee damage Shiva's Guard - reduces attack speed of carries as well as adding armor to survive carries & movement speed for when you get all up in their face
Need a disable? Scythe of Vyse - awesome disable for any hero, use correctly and it dominates
Fast, strong ulti Afghanim's Scepter - makes ultimate stronger and more useful
Need to slow someone more? Eye of Skadi - provides overall great health and mana while adding a massive slow to auto attacks, useful against ranged carries that are trying to auto attack you to death
Super HP tank Heart of Tarrasque - I NEED HEALTH

Late game

Run around with your team and keep your team alive while being that super aoe awesome tank that you're meant to be (even though you're int hehe). Use Reaper's Scythe to finish off important heroes, as it is a great finishing nuke. With this, end the game as soon as possible. Strong carries sometimes become unbeatable and we just can't have that happen now can we? ;)
