The description says the healing effect only applies when "out of combat." How exactly is this defined? Some other items are more specific in that they say the effect is lost if attacked by another hero, tower, or rosh, etc....... What exactly does "out of combat" mean? Is the effect lost if you're attacked by jungle creeps for example?

No, u can activate it while farming jungle creeps, but a hit from an enemy hero, tower or rosh will take the effect away.

As long as you don't take over 20 damage per hit, shouldn't dispel.

The reason why it doesn't dispel is because I doubt creeps can hit more than 20 damage against you after your armor reduction.

For Roshan and heroes, possibly most jungle creeps all will hit for more than 20 so naturally, it gets dispels.

But as far as tested, usually junglers who make vlads like ursa or those who make tranquil (axe) use it while jungling and they have low base armor but even then it doesnt get dispeleed, so i guess neutral camps are not included only enemy units and rosh.
