Nice guide, reading this taught me a lot about playing Visage. But still I cannot get past the 3k8 MMR. I see on DotAbuff some people buy Aether Lens and/or Eul Scepter on him, I also tried that but did not work like it worked in the vids. Maybe you could have a section about using Aether Lens or Eul.
And new players cannot understand the way to micro managing Visage could you make it more specific?

Great guide and all but I'd like to make a few notes:

The item build you are going for is more of a semi-carry rather than a true support...

If you want to be a "support" then take in these following notes:
-Tranquil Boots are one of the BEST boots you can have as a Visage but consider your mana-poor teammates so Arcane is also a viable choice as you have said that his skills take so much mana but low cd...
-Do not forget to buy wards for the team but if you are near to your MEDALLION (200-100 gold away) DO NOT SPEND IT rather save up for your medallion as it will greatly help you and your team in ganks and teamfights...
-Sometimes going for SOLAR CREST first is better than going agh's after medallion.
-NEVER BUY DESOLATOR OR HARD-HITTING ITEMS face it you aren't a hard carry or a hitter... YOU ARE A SUPPORT. AC, Shiva or even Vlad's are better items. (Maelstrom/M'Jollnir is fine tho.)
-If you are in a lead and enemy heroes are dying alot an urn would be great early on.
-LASTLY ADJUST TO THE GAME DON'T BUY AGH'S IF YOU OR YOUR TEAM DIE TOO MUCH... BUY WARDS INSTEAD.

If you want to semi-carry do the same item build but I'd still like to make a few notes about it:
-NEVER GO FOR TREADS as a semi-carry... PHASE BOOTS or TRANQUIL ARE BETTER OPTIONS.
-GO FOR MEDALLION -> SOLAR CREST IMMEDIATELY **** THAT AGH'S ATM.
-Don't go Desolator... It's bad.. seriously... Go for AC instead it's FAR better.
-Hex Stick is really good lategame get it ASAP after Agh's.
-DO NOT, I repeat, DO NOT let the game prolong or else the enemies will get farmed and outcarry you... If you have better or suitable carries than you... SWITCH TO SUPPORT ASAP in Mid-Late game... You fall off mid or late game deal with it...
-Skill build should go 2-1-2-3-2-4-2-1-3-1-4-3-1-3-AP-4-AP all the way~

That's all... I hope you'll improve this guide soon :)

EDIT: Guide is looking good now :) +1 from me :D
